Prior Authorization Policy

If your insurance requires you to have a prior authorization from your primary care doctor in order to be seen by a specialist or to receive a test, please make sure you follow the instructions listed below.

1. Schedule an appointment with the specialist and/or ordered test. We require at least 3 business days for the authorization to be completed, so please schedule your appointments accordingly.

2. Once the appointment has been made, please call our office with the following information:

  • Patient’s full name, date of birth and phone number
  • First and last name of the specialist the patient will be seeing and/or the name of the test and location. Most specialist work from several locations and therefore we cannot fax the authorization if we do not know what location the appointment will be at
  • Date of appointment
  • Reason for appointment

3. The referral you are given from Dr. Ducato is NOT an insurance authorization. You may reach out to your insurance company if you are unsure whether or not a prior authorization is needed by your insurance.

Unfortunately we are unable to perform same day authorizations. If the above steps have not been completed, your authorization will be delayed and may require you to reschedule your appointment with the specialist or facility. We appreciate your understanding.
